چکیده

Study Design. In vivo analysis in an ovine model.Objective. To evaluate the feasibility of radiopaque UHMWPE sublaminar wires in a growth-guidance spinal system by assessing stability, biocompatibility and growth potential.Summary of Background Data. Several growth-guidance systems have been developed for the treatment of early onset scoliosis (EOS). The use of gliding pedicle screws and metal sublaminar wires during these procedures can cause metal-on-metal debris formation and neurological deficits. Novel radiopaque UHMWPE wires are introduced to safely facilitate longitudinal growth and provide stability in a growth-guidance system for EOS.Methods. Twelve immature sheep received posterior segmental spinal instrumentation; pedicle screws were inserted at L5 and radiopaque UHWMPE (bismuth trioxide) wires were passed sublaminarly at each level between L3 and T11 and fixed to dual cobalt-chromiun rods. Four age-matched, unoperated animals were evaluated to serve as a control group. Radiographs were taken to measure growth of the instrumented segment. After 24 weeks, the animals were sacrificed and the spines were harvested for histological evaluation and high resolution peripheral quantitative computed tomography (HR-pQCT) analysis.Results. No neurological deficits occurred and all instrumentation remained stable. One animal died from an unknown cause. Substantial growth occurred in the instrumented segments (L5-T11) in the intervention group (27± 2 mm), which was not significantly different to the control group, (30 ± 4mm, p = 0.42). HR-pQCT analysis clearly showed safe routing and fixation of the UHMWPE wires and instrumentation. Despite the noted growth, ectopic bone formation with the formation of bony bridges was observed in all animals. Histology revealed no evidence of chronic inflammation or wear debris.Conclusions. This study shows the first results of radiopaque UHMWPE sublaminar wires as part of a growth guidance spinal system. UHMWPE sublaminar wires facilitated near-normal longitudinal spinal growth. All instrumentation remained stable throughout follow-up; no wire breakage or loosening occurred and no adverse local tissue response to these wires was observed.

New developments in the treatment of early-onset spinal deformity: role of the Shilla growth guidance system

Early-onset scoliosis is a complex condition with multiple facets. The goal of treating any spinal deformity is to improve the condition of the patient with the least intervention necessary. A system that allows for continuation of natural spinal growth while correcting the deformity should be the goal of treating this complex condition. The SHILLA growth guidance system allows for continued gr...

Augmentation of third generation instrumentation with sublaminar titanium wiring in late onset idiopathic scoliosis: the surgical results and analysis of trunk balance.

In recent years, third generation instrumentation systems which achieve correction by maneuvers like derotation and translation, have been widely used in the treatment of idiopathic scoliosis. To increase correction, additional procedures that increase stability, such as screw application for every segment, have been used. In this study, as a new technique, the effects of combined translation a...

Usefulness of Sacral Sublaminar Wire for Low Transverse Sacral Fractures: Two Cases' Report

Low transverse sacral fractures are rare, with only two published reports regarding their surgery. The complication associated with surgery for sacral fractures is the prominence of implants. In addition, screw fixation below S3 is impractical. We performed posterior sacral fixation using S2 alar iliac (S2AI) screws and sacral sublaminar wires for low transverse sacral fractures. Case 1 was 65-...

Determination of different types of scoliosis based on the apex of the spinal curve in adolescents

Background: Scoliosis is a Greek word for crocked and is an often referred as an shape side to side curves of spine. The spinal column twists and rotates creating an uneven shoulder and a rib hump, which can lead to breathing problems as the heart and lungs became compressed. Early detection, can increase the chances of successful treatment. The treatment of scoliosis varies depending on severi...

Luque rod instrumentation in the treatment of adolescent idiopathic scoliosis. A comparative study with Harrington instrumentation.

Adolescent idiopathic scoliosis in 152 patients was treated by Luque L-rod instrumentation and early mobilisation without external support. This series was compared with a matched group of 156 patients treated by Harrington instrumentation and immobilised in an underarm jacket for nine months. All the operations in both groups were performed by one surgeon and the patients were followed prospec...
